Database Notification Error Svg by name "o-annotation" from set "heroicons" not found.
just install and enable the database notification from panel provider, and got this error Error Svg by name "o-annotation" from set "heroicons" not found.

Solution
I have solved the problems, its caused because the icons from filament 2. i have migrated my apps from filament 2 to filament 3. managed to solve with clearing all database notifications.
