Understanding the Use of ManagedRuntime in Effect Typescript

I've read this, but I don't fully understand when I should use this. Is this just for frameworks? Or should I can I use this for the endpoint of my "service". IE: A custom client. I guess, am I using the "ManagedRuntime" appropriately here?
