Modal fallback not working properly

The agent fallback isn't working properly. When the first model (gpt-4o-mini) hits a quota error, it's not falling back to the second model (claude-haiku-4-5).

What's weird:
An incorrect API key DOES trigger the fallback, but a quota error doesn't. So Mastra's fallback logic isn't treating quota errors as fallback-triggering errors.
