Exec randomly fails with "missing tool result..."
Goal: Normal exec tool usage (shell commands)
Expected: Exec returns results consistently
What happens: Exec randomly fails with:
Simple commands sometimes work, sometimes don't. No pattern.
Logs show:

- 46GB RAM, <1% used by container
