QMD + sessions + plugins: memory_search returns only session transcripts, never memory files
If you use QMD backend + sessions.enabled + custom plugins (before_agent_start hooks), memory_search returns ONLY session transcripts and never your actual memory files.
Root cause: plugin-injected boilerplate (e.g. critical-rules, facts injection) repeats in every turn, inflating BM25 scores for sessions.
Workaround: disable sessions in QMD config (sessions.enabled: false, memorySearch.sources: ["memory"]). This makes memory_search work correctly on memory files again.
