Model fallback issue

We've been investigating why OpenClaw skips fallback models and shows "(unknown)" errors. Here's what we found:
Problem:
• When primary model fails (e.g., rate limit), fallbacks should activate
• Instead: only first 2 models tried, 3rd never attempted
• Error shows: "All models failed (3): minimax/MiniMax-M2.5: LLM request timed out. (unknown)"
Root Causes:
1. Fallbacks skipped - In
2. "(unknown)" shown - CLI runner explicitly sets
Proposed Fixes:
