Even after choosing "Allow Always" I still get exec-approval requests

What I'm seeing
Every execution of /bin/cat via openclaw nodes run --agent main triggers a native macOS approval dialog on the companion app, even though /bin/cat is in the main agent allowlist and ask is set to on-miss. The command runs after approval, and lastUsedAt is updated in the allowlist entry each time.
Every time I run a subsequent /bin/cat I get prompted for approval even though it has been added the allowlist.
