Production outage: FATAL 58030 (pg_filenode.map I/O error)

We’re seeing this error in Dashboard (Auth > Users):
FATAL: 58030: could not open file "global/pg_filenode.map": Input/output error
Our app is also failing DB/schema queries at the same time, so this looks like a DB-level issue rather than app code.
Already submitted a support ticket with screenshots/logs, but posting here in case anyone has seen this and can suggest immediate best steps (failover/restore/PITR guidance).
