yes
the bot only records one vote per (user, message, starboard) - so if there's a downvote, and then an upvote by the same person on the same message, it has to replace it

The bot doesn't actually store the reaction so there's no efficient way for it to know what reactions to remove

there's no way to do that unfortunately, the minimum for "required" is 1
