Custom Prefixes

For the creatives who knows how to make a custom PNG prefix for ranks.
What are the usual size for the prefix png and the dimensions used inside the actual resource pack? Got the gist of how to make a custom prefix, how to put it in a resource pack and put it in as a prefix but I can't seem to make it work in my server and figured that it's probably a size problem. It's only showing the unicode of the prefix.
If you could point me to a blog or maybe a tutorial on how to make this work specifically for prefix ranks that would be amazing!
