Memory Violation Causes Server Crash

In short... I have a custom pack on a server which normally tends to run fine however the server occasionally crashes with memory access violation.
I do see Mekanism Utils putting out a lot of errors in the logs but not sure if that is causing the crash(es) or something else?

The attatched files are all from the latest crash.
