Unresponsive screen randomly and after sleeping

I've struggled a bit recently with one or both screens randomly becoming unresponsive, it fixes itself when I restart my screens but I'd like a more permanent solution.
In addition it often becomes completely unresponsive after entering sleep.

Quite new to linux and not very good at troubleshooting, so any help is greatly appreciated :)
