Upgrade from 2025-01 to 2025-04 resulted in "Request access to Protected Customer Data"

I recently upgraded from 2025-01 to 2025-04 and now am getting "Request access to Protected Customer Data" but I already have an approved form. Any suggestions how to fix this?
