Trigger skipped conditions and started banning people

Condition: Has teamkilling flag

Add BM Ban

Remove teamkilling flag
The trigger ignored the one and only condition and instantly banned around 30 people who did not have, and have never had the teamkilling flag.
Since all actions and conditions are from within BM, and do not involve the game server (except for triggering on Player Update), I'm at a loss at what could have caused this. The trigger has worked as intended for almost a year, it only malfunctioned like this one other time in August, and we asked about it in #support but didn't get a response.

@Glocko teamkills are name only rcon messages from the server, no unique ID.
Was any of the people removed a 1 or 2 character name?
Nope, all normal names selected seemingly at random
Also the trigger type is Player Update, not Teamkill. The "ban wave" was not preceded by any teamkills
Does the flag get removed from the ban or stays?
None of them have ever had that flag
That's the whole problem
Only hordicus would be able to look through, he would need a Ban ID or Message ID to look back into the logs
can I ping him?
@Hordicus can you review the logs to check for Miss firing on trigger, appears it bypassed a condition and banned multiple.
