Distant Horizons is preventing connections to my server.

I regret that I have found myself in this situation, and I am unsure of how I've arrived in this position. My server is new; I opened it within the week. As I ported my long-standing hardcore world over, there was only one issue on startup: DH felt the databases were corrupted, so I deleted them. The server ran great for the first days.
When my daughter and I returned to The End to reacquire her elytra, I noticed DH appeared to not be sending LoDs. This prompted me to begin tampering with settings, to no avail. However, it does appear we may have started receiving LoDs after several client restarts.
Here is my current situation: If I disable DH (by disabling it's *.jar,) then my server runs fine. If I reenable it, I am able to connect, but I immediately time out. Thus far, I have deleted DHs config file to restore defaults, and I have deleted DHs SQL databases. With DH enabled, it appears the easiest form of relief is to drastically reduce common.playerBandwidthLimit (to double digits, down from the default of 500.) This inofitself blows my mind, as my vendor, Apex, advertises "no limit to bandwidth," (obvious caveats applied,) and I did not think 500 kB/s would be that excessive for a server only scheduled for four players.
Are there recommended server settings that I missed?
