Bug: Session pruning corrupts tool history
Long sessions with many tool calls eventually fail with:
Looks like conversation pruning (to fit context limits) removes tool_use blocks but leaves orphaned tool_result blocks, causing Anthropic to reject the request.
Workaround: /new to start fresh. But the pruning logic should probably keep tool_use/tool_result pairs together.
