I was hacked 6 months ago via a phishing site. I have all my original purchase receipts (GPA numbers) and original device info, but the in-game form keeps asking for teammate info from the last 6 months which I don't have because the hacker was using the account.

To recover your account, please submit a recovery request through in-game by going to the login page and selecting:
Help > Hacked Account & Other Account Issues > Account Recovery.
