Do I need to give permission to use the game?
When I try to play say Vampire survivors I get a endlessly loading screen. Every time I launch the game this pops up in my browser. Do I need to say yes to get the game to work or is saying "yes" not necessary?
I don't ask for just this game but a number of games

they games are just asking to associate with your epic account

I think I might have said no for other games, you know where to go to fix that?
Pretty sure they're just gonna open the prompt again next time you launch one

i mean you have to accept to play.. im not sure this is an issue? its basically: read the pop up, allow or dont play.

I mean if you set a game to not allow, it won't come up again, correct? In that case how do I change it?
Also does this mean I need to be logged in to Epic Games in my browser to install new Epic games in Heroic?

you don't need to be logged into epic games in the browser, once a game is authorized it shouldn't ask to do that again (except for some games that implement that wrong and ask you every single time)
honestly, I don't think it's worth adding to the wiki or the read me first, today is the first time I read this question in the last 3 o 4 years, it's basic terms of service and it's not a heroic thing, it's how epic works
