if RTSS hooks into DirectX/OpenGL/Vulkan to overlay info or if Vanguard detects RTSS’s hook as a code injection or overlay, it could be flagged as suspicious if RTSS is outdated or you’re using custom RTSS plugins or modify RTSS config manually
that’s just my opinion though nothing proven
