What could even possible cause something like this?

Try turning off the renderer and turning it back on in the Distant Horizons setting.
I would guess its some mod causing a issue

Try a binary search and figure out which mod is causing it.
So disable half of the mods, see if issue is still there, if yes disable half again etc etc until you find the mod that's causing the issue.
Yeah there was a mod that does this. Forgot which
It fixed itself with deleting the sqlite and restarting the game, but still very strange lol
