As Arthie mentioned, this was necessary when D1 first came out, I too had the binding setup ever since then, never updated it. Issue for ref - https://github.com/cloudflare/workers-sdk/issues/2402

i can get this prefix too, in toml and dashboard, i set DB binding, but when i log the env, there is a prefix __D1BETA , i use wrangler@3.4 solved problem

Generally no. Images should go in some kind of object storage, like R2.
Once you've stored them externally you can keep a link or the path to the image in your database.

I would store them in R2 and keep a reference in D1. Your max row size will limit you.
