The second option seems to work. But I need to add them programmatically for each customer willing to have a custom hostname. I found the API doc to add a worker route https://developers.cloudflare.com/api/operations/worker-routes-create-route. That should work. But I was wondering if it is the right solution.

it's not really the right solution no, there's a limit of 1000 routes or so and if you get too many they slow down overall routing (each one has to be evaluated)

the right solution would just be a wildcard like they suggest, then using a single wildcard to exclude everything on your domain

it is a bit messy though and potentially confusing for you in the future, you could buy a second domain dedicated to your customers/cf for saas to better isolate it
