High level, the chip is failing to power up:
- Do you have a voltage setting jumper in the programmer? Check it and set to 5V if you're using a lower level.
- Power both Vcc and AVcc with the same level(recommended even if you're not using ADC)
- Connect to all the GND pins
- Physically verify the pin voltages
- Just noticed, also the RESET is active low - meaning it's state should be held high to keep it in operating mode. Verify if that's happening.
