High level, the chip is failing to power up:
  1. Do you have a voltage setting jumper in the programmer? Check it and set to 5V if you're using a lower level.
  2. Power both Vcc and AVcc with the same level(recommended even if you're not using ADC)
  3. Connect to all the GND pins
  4. Physically verify the pin voltages
  5. Just noticed, also the RESET is active low - meaning it's state should be held high to keep it in operating mode. Verify if that's happening.
