You can use https://github.com/nora-soderlund/cloudflare-d1-backups to backup your database and then download that. There is currently no built in way

You could create your own worker that routes your sql queries or you can use https://github.com/elithrar/http-api-d1-example (I have not used it for a while so I'm unsure if it still works)

For instance sqlite doesn't support adding a foreign key column to an existing table, so you need to create a new table and migrate.

No, because it deeply depends on query complexity, schema, etc. It’s not 1:1. You can write millions of rows per second in many, many cases.
