You could create your own worker that routes your sql queries or you can use https://github.com/elithrar/http-api-d1-example (I have not used it for a while so I'm unsure if it still works)

For instance sqlite doesn't support adding a foreign key column to an existing table, so you need to create a new table and migrate.

No, because it deeply depends on query complexity, schema, etc. It’s not 1:1. You can write millions of rows per second in many, many cases.
