Unable to fill claim form

I'm unable to fill out a claim form for my hacked account as it says "failed to fetch data, please try again later (30400101)" everytime I've tried filling out the form in the past 3 months. I kinda forget to comeback and try again in a while but everytime I comeback it shows this error.

I'm assuming it's getting rejected as I haven't had access to the account in over a year and the recovery form requests information from the past 6 months
