I've tried async for 1.21.1 Fabric, though the inconsistency and about 2-5ms higher mspt, I think its a pretty good concept

Fabric are somewhat hard to be fully optimized, normally you can just pick some mods like c2me, lithium,...
But as far as I know, those mods only provides partial multithreading methods, pairing with other content mods and you'll find it even worse in terms of mspt.

Have you tried switching the nameservers to CloudFlares ?
IFAIK many Namecheap users tend to do that because CloudFlares has better domain "managing" features than Namecheap
